Then, in September 2009, the Songjiang District Quality and Technical Supervision issued a safety inspection directive to the company involved, ordering it to stop using the wire rope electric hoist. Despite this, the company continued to use the equipment for an extended period. Zhou Moumou and Qian Moumou, who had been responsible for inspecting companies in their jurisdiction since 2004, failed to take the necessary actions. On August 27, 2010, during an inspection of the incident company, they discovered the illegal use of the hoist but did not report the violation to the local quality supervision authority. Instead, they simply issued an inspection report and left the matter unresolved. Following that, the two coordinators neglected to conduct thorough inspections of the company’s production areas during subsequent visits. They merely recorded the equipment usage as normal in the "Monthly Inspection Report" submitted to the district authorities. Their lack of diligence allowed the company to continue operating with dangerous machinery, which eventually led to the tragic accident. This case highlights the serious consequences of failing to uphold safety responsibilities. It serves as a reminder of the critical role that regulatory officials play in ensuring public safety and preventing preventable disasters.
